IMO it is NOT an unnecessary scene.

It establishes Shanmugam's character very well.
He suspects that Mohana is no different from the rest and is trying her charms on Singapuram minor. This was based on flimsy evidence. Even when he goes to play the ThillAna in Thiruvaaroor, we have no reason to believe his suspicions have gone away.
But the suspicion seems to magically vanish at the end of the contest when he announces the title: "thillAna mOhanAmbAL".
This again is attributable to his mercurial personality and his artistic temperament.

The nurse scene is not bad at all. Sivaji does really well in the scene with the nurse.
The expression on his face when he has the thermometer in his mouth is
The way he says "enakkAga nee romba sramam eduthukriyOnnu.." is just beautiful.
The only thing which I don't like about the scene is the cliche that he sees Mohana in the nurse.

Note: he does not spurn only the nurse's perceived advances in the film. Jiljil physical innocent physical proximity is also deftly avoided by Shanmugam even in Azhagar kOvil.

That Shanmugam's talent is known even outside the world of traditional music. An obscure church choir organ player (possibly much older than Shanmugam) respects his talent so much that he speaks highly of him to his daughter.

But the daughter is able to size his character up accurately, saying that all he seems to know is to play the Nadaswaram and that is it.
(which is a strong lashing to the ego than the more soothing generalization: "kurai illaadha manushan Edhu jillu ?")

That these minor actions of hers are being misread by him is a realization that should have stung the suspicion-ridden man that Shanmugam is. Hadn't he suspected Mohana based on even flimsier evidence ?

I don't think this scene shows him in poor light. It shows him in flesh and blood as the hyperconservative man who gets a lesson that his talent does not overcome his other shortcomings. But his jealousy - which is the signature of his love - stays on even after he gets emotional in this event. That is a real characterization
