Yes that's a very famous dialogue. My favorite consulting scene is the token number previous to this patient where patient mentions about a problem and then Gounder takes the lead from there to quote 3 other symptoms (Kannu tappu tappunu adichikudha, mutti valikudha... etc). He builds up the expectation in such a way that patient is quite confident that doc will provide cure for all his ailments, now kounder does a complete u - turn (like Soru illa poda) and then says... 'idhellam enakku irukku... onnum panna mudiyaadhu'. :-) :-)
Digression
As I typed this, it reminded me of explanation of laughter (from a evolutionary context) provided by Dr.V.S. Ramachandran in a book by name Tell tale brain.
Any joke or humorous incident has the following form. You narrate a story step-by-step, leading your listener along a garden path of expectation, and then you introduce an unexpected twist, a punch line, the comprehension of which requires a complete reinterpretation of the preceding events. But that's not enough: No scientist whose theoretical edifice is demolished by a single ugly fact entailing a complete overhaul is likely to find it amusing. Deflation of expectations is necessary, but not sufficient. The extra key ingredient is that the new interpretation must be inconsequential. Let me illustrate. The dean of the medical school starts walking along a path, but before reaching his destination he slips on a banana peel and falls. If his skull is fractured and blood starts gushing out, you rush to his aid and call the ambulance. You don't laugh. If people don't look beyond GM's slapstick, then I really pity them. I am sure a threadbare discussion would have happened about his style of humor, but I haven't gone thru it yet. In my point of view, slapstick is used right from Tom/Jerry to what not... Charlie Chaplin/Vadivelu used themselves (self inflicted) to evoke laughter and our man used Senthil.
In essence, the brand has universal appeal, so why bother even to discuss about it...
More importantly, Charlie Chaplin uses the way he falls/hurts himself to invoke laughter, however, it has seldom been the case in GM/Senthil. It's always.. konjam thirumbu and then a KICK. Actually, we are more interested in the context of the 'kick' rather than the kick itself.

Laughed uncontrollably when I read this. Got some strange looks as I was on the middle of the road then. Only a Gounder devotee can understand.
After dog bits annan, vaithiyar advises to take care of the dog for 14 days... during the sequence... he says something in the lines "mapillai andha 14vadhu naal mudiyattum..apparam paapeenga..." seamless shift in modulation and emotion. Remember?? One of my favorites from the film.

Good that you can recollect the line. Have seen PR list before, yes this one and many more can make to that list.
Remember the scene from 'Neengalum Herodhaan' where both GM/S ரொம்ப பணிவா gets introduced to the filming crew and the moment GM finds his feet, changes his modulation/body language in no time from பணிவு to one bordering arrogance and asks 'கதை நல்ல கதையா'... ROFLMAO moment.
